In the new default combo table, the right side of the South Locked door for the Outlands Doors is set to (None), and the right side of the South Boss Door for the Outlands Doors is set to No Fly Zone.
R. side of South Outland Locked and Boss Door Combos are not defined right by default
Oops! I double and triple checked for those sorts of mistakes too. Given how simple the fixes needed are, this might not be helpful, but in the event that patching in a new .qst is easiest, here's a fix:

I also fixed the animated waterfall thing (I'm pretty sure that was pre-existing) to kill two birds with one stone.
EDIT: I replaced the link with one that fixes all the blank items with non-gameplay items. A few were Whistles which was causing some problems.
