I used to think it was the one on the right too, but at some point I realized the white spots were eyes. I am not sure what made me realize it. It was probably when I took a closer look to how he was depicted in the comic series that ran in NP. Compare the image and you can see some resemblance.

I think the problem is that Agahnim's head is positioned too low, and the turban+circlet hat combo is too large.

I always pictured the one on the left, but I've had this problem with other games. For example, in FF4 I always thought the Goblin's sword was actually its nose. Probably should have taken a closer look at those pixels!
The image on the right distrubs me, yet also makes me laugh.

Holy shit. Wow, till seeing this, I totally thought it was the one on the right (sans the long hair, I thought it was a hood). I've seen Agahnim's artwork, and knew he was supposed to have have a burka looking thing, and I never could figure out, why one, he was green, and two what was going on with what I thought for years was his face/ head.
This makes a little more sense of it. By looking at him (and later, Ganondorf) one has to wonder why the Hylian kings trust such sketchy looking motherfuckrs.

I never saw it that way, but looking at it now... Kind of looks like Porky/Pokey, doesn't he?
