The death counter for a Quest only goes up if the player selects "Continue" on the game over screen. This really isn't a bug, but it would be nice if it was made so there is an option to have the death counter go up when a player selects "Save" or "Retry".

The main reason this is an issue for my quest is it uses the "Skip Continue Screen" and the "No Save When Continuing" Quest Rules (For there are save stations in the game itself), and those would go nice with a "Increase Death Count When Retry Or Save Is Chosen" quest rule.
