GTA3s goast town
going to Garzhad's fotress(Exile II) for the first time, the way the level's were layed out, with the arrangement of enemies, the teleportations, and then the hidden traps
expecially in the room before Garzhad; when you walk in it's empty, then blade clouds appear to damage you with every step; suddenly demons appear; then super-demons; with no stop until you get into the throne room(which is immaculant)
never encountered a place like that before...
All of Quake1 was like a weird drug-induced nightmare.

Dunno if this counts, but in C-Dawg's ZC Quest Dreams of Yesterday, Error's nightmare, "Gallery," was one of the freakiest places I've ever had the pleasure of killing things in. There were rivers of blood, invisible enemies that scared the living hell out of you, and odd puzzles such as when you walk toward a particular key, it would suddenly appear behind you, and when you had to find a way to get to that key, it would just appear behind you again. .. So f*cking freaky. :p .. Damn impressive, C.
The most mysterious area I seen in a game would be in Bloodrayne, where the Nazi's had a bunch of piled corpses of jews on a puddle outsite of a mining factory, it seem eerie because as soon as you get there you can see them in the puddle and there odd expressions of horor.
Castlevania: Symphony of the NIght had the catacombs... The music itself just haunts my head, not to mention... all the dead bodies and skeletons. All of which are in their funeral attire, and look like they can jump straight from the background in Alucard's path.
I'd also have to say... the desert stage where you fight Dracula's third form in Castlevania 64, the final level in Simon's Quest, the well in Ocarina of Time, the moon area of Majora's Mask, and anywhere in Eternal Darkness.
