you have to input your name, then either hit enter for classic quest, or 'a' for a custom quest...

try reading the FAQ (instruction guide)

by the way, once you die and save the file, you MUST delete it before you can have a different custom quest in that file. (hint: use zeldaclassic prog to delete the file, and not trashing it yourself.)
