A quest shouldnt be based around difficulty OR puzzles. The designer should have his image in mind, and then shape his game to match that image. Difficulty and puzzles just happen naturally along the way... so I guess a better question is, do you design quests by thinking about what sort of neat puzzles to make, or do you decide what you want to do and then make ZC your bitch?
