On level 4, the ladder will be located on the lower right corner of the "D" shaped map.

Level 4 entrance will be located one screen above where level used to be, It is a screen left on the left side of the narrow river and it is a bombable wall.

again, sorry to double post, all I need to find now is the bracelet and the blue ring

The blue ring will be in level 5 where the red potion and heart container used to be at on an island where you ride the raft on the east coast.

The bracelet will be in level 4 on the left side of the "D" shaped map.
