Doors keep relocking themselves!

Uh I have a bit of a problem, sometimes when i unlock a door and i die and come back again, its locked again, can somone explain this to me.

P.S. This doesn't happen every time, just sometimes.

Try looking at the screen flags and see if there is some weird one checked.
