what is your OS?
How fast is your cpu?
fullscreen or windowed?
What quest file are you playing?
What color is you monitor?
Is this too many questions?

Try toggling your vsync in the menu options. It may also be called throttle or something like that. F1 may be a hotkey for it. See if just changing that option fixes your issue.

Your OS is like windows 98, XP, etc.

And yeah, try messing with VSync. That's often it.
