How do -you- construct your combo pages?

Suppose you had to start your ENTIRE combo page anew. How would you do it?

For example, how much space would you reserve? Would you put everything in a specific place initially as your quest is planned out entirely, or would you just add combos as you need it, filling it up from the top down? (Not very organized, [AT ALL, actually.] But it's certainly easier than having to make more room for stuff. :p )

I construct combo pages on a need to need basis.

When I was working with Orion's LTTP Tileset for Days of Eternity, all the combo pages were pretty much from scratch. I added things I figured I'd need as I needed them. I do try to have a little organization, for my sake, but there isn't much. :shrug:
