Problem with database? Register not working...

A friend tried to sign up here. He gets this error:

Additional note: He refreshed multiple times and tried hitting back and submitting again.

i'm having the same problem with newbie central. maybe if i'm lucky, an admin will show me how to fix it while it's fresh in his mind. :D
