anybody who's currently under 18 and planning to attend the convention needs to have this form filled out and notarized. i have to have a copy of it in my hand before the 20th. you can not just bring it with you to give to me. that doesn't help me in you're in a car wreck on your way to the convention. you also need to have a signed and notarized copy with you, giving medical personnel information and permission to treat you should you become sick or get injured on the way to or from the convention. that's two copies...both notarized. one for you and one for us. the file is here (http://www.soulfactory.com/convention/consentform.pdf) in .pdf format. it's two pages long.
remember...signed and notarized and in my hands no later than june 20, 2002
